Pursuant to the Regulations on the Administration of Fishing Vessel Crews Articles 5 and 16.
Matters for promulgation:
- 1.The format of application form for fish worker’s license of foreign crewmembers on fishing vessels of the Republic of China is shown as in Annex.
- 2. When applying for reissue of fish worker’s license of foreign crewmembers due to mutilation or loss, his vessel owner shall submit a copy of the application form, accompanied by the mutilated license or newspaper advertisement declaring loss of license, two recent photos of 2.5” shoulder up, without wearing hat, in accordance with Article 16 of the Regulations on the Administration of Fishing Vessel Crews.
